Official Rules​

1. NO PURCHASE OR PAYMENT OF ANY KIND IS NECESSARY TO ENTER OR WIN THIS PROMOTION.  A PURCHASE WILL NOT IMPROVE YOUR CHANCES OF WINNING.  ALL FEDERAL, STATE, LOCAL AND MUNICIPAL LAWS AND REGULATIONS APPLY. VOID WHERE PROHIBITED. 
2. Sponsor: The Grapevine Health Sweepstakes (the “Promotion”) is sponsored by Grapevine Health (the “Sponsor”). 
3. Official Rules: These are the official rules (the “Official Rules”) for the Promotion. Participation in this Promotion is expressly subject to and governed by these Official Rules.  By entering the Promotion, each participant (“entrant(s),” “participant(s),” or “you”) agrees to be bound by these Official Rules and the Sponsor’s decisions, which are final and binding in all matters related to this Promotion, and to comply with all applicable rules and regulations.  Sponsor has the right to reject and disqualify you for any reason, in its sole and absolute discretion. Winning a prize is contingent upon fulfilling all requirements set forth herein. 
4. Promotion Period:  The Promotion will commence on June 5, 2022, at 12:01 a.m. Eastern Time (“ET”) and end on July 30, 2022 at 5 p.m. ET (the “Promotion Period”).  The Promotion contains a game of chance, and the computer clock of the Sponsor will be the official time-keeping device in the Promotion.
5. Eligibility: The Promotion is open only to legal residents of Maryland, Virginia, or the District of Columbia, who are 18 years of age or older as of date of entry. Employees, officers and directors of the Sponsor, Lisa Fitzpatrick & Associates MD PC d/b/a Grapevine Health, as well as their affiliates, subsidiaries, and their advertising, contest, fulfillment and marketing agencies (collectively, the “Promotion Parties”), as well as their immediate families (parent, child, sibling & spouse and their respective spouses, regardless of where they reside) and persons living in the same households as such individuals (whether related or not) are not eligible to participate in the Promotion. Void in where prohibited by law, rule or regulation. All federal, state and local laws and regulations apply. 
6. How to enter:  There are two methods of entry:
   1. Entry in Person at a Discussion Group: 
      1. There will be four (4) discussion groups held during the Promotion Period at which attendees will discuss how to make health programs more fun and interesting (each, a “Discussion Group,” and collectively, the “Discussion Groups”):
         1. Discussion Group 1 will take place on [ENTER DATE] from [ENTER TIME] at [ENTER LOCATION].
         2. Discussion Group 2 will take place on [ENTER DATE] from [ENTER TIME] at [ENTER LOCATION].
         3. Discussion Group 3 will take place on [ENTER DATE] from [ENTER TIME] at [ENTER LOCATION].
         4. Discussion Group 4 will take place on [ENTER DATE] from [ENTER TIME] at [ENTER LOCATION].
      2. Attendance at a Discussion Group will result in one (1) automatic Entry.  You must attend the full Discussion Group session to be entered.  Attendees who arrive late or leave early will not receive an Entry.
      3. Limit of up to one (1) Entry per person throughout the Promotion Period, regardless of the method of entry.  Attendance at more than one Discussion Group will not result in multiple Entries.
   2. Entry via Mail:
      1. You may enter via mail by printing your full name, address (including city, state, and zip code), e-mail address (if you have one), and phone number on a 3″ x 5″ card and send to: Grapevine Health, P.O. Box 8802, Silver Spring, MD 20907-8802
      2. No mechanically reproduced or photocopied entries accepted.  Entries become the property of the Sponsor and will not be returned. 
      3. Mail entries must be postmarked by 4:59:59 p.m. ET on the following dates for the respective drawings:
         1. Drawing 1: June 16, 2022
         2. Drawing 2: June 30, 2022
         3. Drawing 3: July 14, 2022
         4. Drawing 4: July 28, 2022
      4. Each valid entry card received via mail will result in one (1) Entry. Limit of up to one (1) Entry per person throughout the Promotion Period, regardless of the method of entry.
      5. Entries that do not include all required information and adhere to these Official Rules as determined by Sponsor, in its sole discretion, will be considered void. Sponsors may, in their sole discretion, remove, delete and/or disqualify any Entry and/or Entrant it deems inappropriate or otherwise non-compliant. Sponsor is not liable for entries that are lost, late, misdirected, incomplete, garbled, incompletely received, mutilated, or illegible, for any reason.
7. Sweepstakes Drawing / Odds: There will be four drawings (one (1) for each Discussion Group), with one (1) Winner for each drawing. Following each Discussion Group, a winner will be selected in a random drawing by a representative of Sponsor, whose decision will be final and binding in all respects.  The odds of winning a prize depend on the number of Entries received.
8. Winner Notification: The potential winners will be notified in person, via email, or by phone (as applicable) within five (5) business days of winner selection or as soon as reasonably practicable. Potential winners will have three (3) days to respond to the notification that they have won before forfeiting their prize. Potential winner becomes a “Winner” only after verification of eligibility by a Sponsor. If such potential winner cannot be contacted within a reasonable time period (three (3) days), if the potential winner is ineligible, if any notification is returned undeliverable, or if the potential winner otherwise fails to fully comply with these Official Rules, he/she will forfeit that prize and an alternate winner will be selected from among all remaining entries for that drawing.
9. Prize: There are four identical Prizes, one for each sweepstakes drawing.  Each Winner will receive four (4)  Thrill Seeker annual passes to Six Flags America for the 2022 season. The approximate retail value of each Prize is $396 ($99 per season pass), and the approximate cumulative retail value of the Prizes is $1,584. This Promotion is not sponsored, endorsed, administered, associated with, or certified by Six Flags America.  The odds of winning a Prize depend on the number of Entrants. No substitution of a Prize, including cash substitutions, will be permitted, except that Sponsors have the right in their sole discretion to substitute a prize of greater or equal value if the stated Prize is not available. Prizes are not transferable or assignable. Limit of one (1) Prize per person/per household. Within at least ten (10) days of Winner verification, Sponsor will ship or otherwise deliver the Prizes to the Winners using the address provided by each Winner during the Winner verification process, using such delivery method and carrier as selected by the Sponsor. 
10. Taxes: Income and other taxes, if applicable in your jurisdiction, on the value of the prize are the sole responsibility of the Winners.
11. Representations and Warranties:  By participating in the Promotion, you represent and warrant as follows:
    1. All information provided by you in connection with this Promotion is truthful, current and accurate; and
    2. You meet the eligibility requirements set forth in these Official Rules.
12. Additional Conditions of Participation:  By entering this Promotion, you agree to forever release the Promotion Parties from any and all actual and potential, known and unknown, suspected and unsuspected claims, demands, causes of action, liabilities, losses and damages (collectively “Claims”) arising out of or relating to this Promotion or any Prize, including, without limitation, Claims for personal injury or death, damage to or loss of property, violation of right of publicity or privacy, defamation, libel, and false light, whether intentional or unintentional, and whether under a theory of contract, tort (including negligence), warranty or otherwise.
13. Limitation of Liability:  THE PROMOTION PARTIES SHALL NOT BE LIABLE FOR DIRECT, INCIDENTAL, CONSEQUENTIAL, INDIRECT, SPECIAL, EXEMPLARY OR PUNITIVE DAMAGES, OR LOST PROFITS, ARISING OUT OF OR IN CONNECTION WITH THE PROMOTION, WHETHER IN AN ACTION IN CONTRACT, TORT (INCLUDING NEGLIGENCE AND STRICT LIABILITY) OR OTHERWISE, AND IRRESPECTIVE OF WHETHER EITHER PARTY HAS BEEN ADVISED OF THE POSSIBILITY OF ANY SUCH DAMAGES. Some jurisdictions do not allow limitations or exclusion of liability for incidental or consequential damages, and therefore this limitation of liability may not apply to you.
14. Winner’s List: The Winners’ names will be available for a period of sixty (60) days beginning thirty (30) days after the Promotion Period has ended. To obtain the Winners’ names, please send a self-addressed stamped envelope to Grapevine Health, P.O. Box 8802, Silver Spring, MD 20907-8802. 
15. Applicable law: Except where prohibited, all issues and questions concerning the construction, validity, interpretation, and enforceability of these Official Rules, or the rights and obligations of Entrants, Winners, or the Promotion Parties in connection with this Promotion, shall be governed by, and construed in accordance with, the laws of the District of Columbia without giving effect to any choice of law or conflict of law rules or provisions, which might otherwise cause the application of the laws of any jurisdiction other than the District of Columbia. Any action seeking legal or equitable relief arising out of or relating to the Promotion or these Official Rules shall be brought only in the courts of the District of Columbia. You hereby irrevocably consent to the personal jurisdiction of said courts and waive any claim of forum non-convenience or lack of personal jurisdiction they may have.
16. Privacy: Except as noted in these Official Rules and except as in connection with the Discussion Groups, information collected solely for entry in this Promotion is used only for the purpose of Promotion administration and winner notification and will not be re-used, sold or shared in any manner by the Promotion Parties or any third parties unless you have elected to receive additional information or promotional material from the Sponsor or a third party.
